Carpal Tunnel Syndrome
Carpal tunnel syndrome is a common condition that affects the hand and wrist, often causing pain, numbness, and weakness. It can make everyday activities like typing, gripping objects, or even sleeping uncomfortable. Carpal tunnel syndrome occurs when the median nerve becomes compressed as it passes through a narrow space in the wrist called the carpal tunnel.
The carpal tunnel is made up of bones and ligaments and protects the median nerve along with several tendons that help your fingers move. When swelling or pressure builds up in this area, the nerve does not function properly. This leads to the symptoms commonly associated with this condition.
The median nerve plays an important role in hand function. It provides feeling to the thumb, index finger, middle finger, and part of the ring finger. It also helps control some of the muscles at the base of the thumb. When the nerve is squeezed, these functions can be affected, leading to both sensory and motor problems.
Carpal tunnel syndrome does not always have a single clear cause. In many cases, it develops due to a combination of factors that increase pressure on the median nerve.
Repetitive hand movements are one of the most well-known contributors. Activities that involve frequent bending of the wrist or gripping, such as typing, assembly line work, or using hand tools, can irritate the tendons and cause swelling in the tunnel.
Certain health conditions can also increase the risk. Diabetes, rheumatoid arthritis, and thyroid disorders are known to affect nerve health or cause inflammation, which may contribute to carpal tunnel syndrome. Fluid retention, especially during pregnancy, can temporarily increase pressure in the wrist as well.
Wrist anatomy can play a role. Some people naturally have a smaller carpal tunnel, which leaves less room for the nerve and tendons. Past injuries such as wrist fractures or sprains may change the structure of the tunnel and lead to compression.
Other risk factors include age and gender. The condition is more common in adults and tends to occur more often in women, possibly because their carpal tunnels are generally smaller. Obesity and a sedentary lifestyle may also contribute by increasing overall inflammation in the body.
Symptoms of carpal tunnel syndrome usually develop gradually and may come and go at first. Many people notice discomfort during activities or at night.
Numbness and tingling are often the earliest signs. These sensations typically affect the thumb, index finger, middle finger, and part of the ring finger. Some people describe it as a “pins and needles” feeling. It may wake you up at night or occur while holding a phone or steering wheel.
Pain can range from mild to severe. It may be felt in the wrist and hand, but it can also travel up the forearm toward the elbow. The pain is often worse after repetitive use of the hand.
Weakness in the hand is another common symptom. You might notice difficulty gripping objects, dropping items more often, or struggling with tasks like buttoning a shirt. Over time, the muscles at the base of the thumb may shrink if the condition is not treated.
A feeling of clumsiness or reduced coordination can also occur. Simple movements may feel less precise, making everyday tasks more challenging.

A physical exam is an important next step. They may check for areas of tenderness, test your hand strength, and evaluate your sensation in the fingers. Specific wrist positions or gentle tapping over the nerve may be used to see if symptoms are triggered.
If further confirmation is needed, diagnostic tests may be recommended. Nerve conduction studies are commonly used to measure how well the median nerve sends signals. Slower signals can indicate compression. Electromyography may also be performed to assess the electrical activity of the muscles and identify any nerve damage.
In some cases, imaging studies such as ultrasound or X-rays may be used to look for structural problems, such as arthritis or a past injury, that could be contributing to the symptoms.
Early diagnosis is important because it allows for more treatment options and can help prevent long-term damage. If left untreated, carpal tunnel syndrome can lead to permanent nerve changes and loss of hand function.
